CDC Healthy Works: Health System Interventions and Community Linkages with Pharmacists

SDPharmNet™ is conducting this project, in collaboration with the San Diego County Health Department and Be There San Diego, to assess and implement pharmacist-provided services for patients with hypertension and diabetes in health-system and community pharmacies.
